The establishment preparation was approved by the Ministry of the Interior in October 2021, and the first preparatory session was convened in December to recruit members.

The second preparatory session was held in January 2022 to finalize a roster of 107 members, which was then submitted to the Ministry of the Interior for future reference.
Feb.2022
On February 20, 2022, the first Society conference was held at the Zhan Hua Garden Hall, where the first chairman was elected.
Mar.2022
The society was officially established with the Ministry of the Interior’s Tai-Nei-Tuan-Zi No. 1110011936 letter dated March 17, 2022.
Missions
The society aims to facilitate graded and integrated healthcare for chronic conditions, with the ultimate goal of improving public health. The society operates in accordance with applicable laws and regulations to achieve the following missions:
Promote graded and integrated healthcare for chronic conditions such as pulmonary obstruction, asthma, pulmonary fibrosis, diabetes, chronic kidney disease, hypertensive heart disease, and heart failure in Taiwan.
Enhance the quality of primary care for chronic conditions and the dual referral collaboration system in Taiwan.
Promote the establishment of a long-term healthcare system for chronic conditions and improve continuity of care.
Improve health education and overall public health awareness in Taiwan.
Develop preventive healthcare and integrated care.
Promote smart healthcare and health industry value chains.
Organize continuous education and training for long-term care practitioners.
Serve as a provider for research and implementation of management policies for long-term care institutions.
Assist members in drafting policies for long-term care institutions.
Promote the development of industries related to telemedicine and continuity of care.
Assist authorities in establishing policies related to graded healthcare and long-term care.
Undertake other activities that align with the Society’s missions.
